A medical certificate is issued to all drivers as confirmation of their fitness to drive vehicles, and often contains special recommendations or restrictions on driving (for example, the mandatory use of glasses or contact lenses while driving).
Medical certificates are issued by clinics that are licensed to conduct medical examinations, including driving examinations. To visit the commission, you only need to present your passport, pay for the services of medical workers and, if necessary, provide a certificate of disability or conclusions of previous medical commissions.
What is the validity period of a medical certificate? As practice shows, drivers are often confused about these terms, and there are a number of reasons for this.
What is a medical certificate?
The format of the medical certificate was determined back in 2010 by the relevant Order of the Ministry of Social Development of the Russian Federation No. 831-N, but at the moment it has undergone a number of changes:
- The certificate was significantly reduced and began to fit on an A5 sheet of paper. This became possible due to the fact that detailed doctors’ opinions began to be entered into a personal medical record;
- The certificate has been supplemented with all new categories of tertiary education;
- It became possible for most doctors not to go to their place of registration, but to their actual location. However, some specialists (for example, a narcologist and a psychiatrist) still have to be seen at their place of registration;
- Restrictions on driving vehicles began to be indicated opposite strictly defined categories of driving licenses. The field for recording the data of the doctor who completed the final issuance of the document was also reduced;
- Certificates are currently strictly accountable documents, so cases of forgery have decreased significantly. They are made on special paper with printing security features.
Validity of medical certificates for drivers in 2021
The medical commission is called upon to assess the health status of a person who intends to drive. It decides whether a person can drive a vehicle without risking becoming a danger to passengers, pedestrians and other road users. The most recent document regulating the work of medical commissions for drivers is Order of the Ministry of Health dated June 15, 2015 N 344n.
The order significantly “tightened the screws” with regard to driver’s medical examinations, practically eliminating the possibility of obtaining fake documents, which used to be a serious problem. Since July 2021, a new driving license form has been introduced, equipped with several degrees of protection.
From July 1, 2021, the validity period of a medical certificate is one year from the date of issue for drivers of all categories of vehicles. The date of issue of the document is indicated on its front side.
The new medical certificate form is well protected from counterfeiting
Previously, Russian legislation provided for the validity of a medical certificate at two and three years. It directly depended on the driver’s age, his driving experience and several other parameters. All certificates issued up to June 30, 2021 continue to be valid until their expiration date. But you will have to go through the new commission under current conditions and receive a certificate with a one-year period.
How long is a certificate from a narcologist and psychiatrist valid?
Passing a driver's medical examination involves visiting a narcologist and a psychiatrist. Their certificates are separate documents (medical certificates of health). They must subsequently be given to a therapist who is the chairman of the driver’s medical commission. The validity of this document is also limited to one year from the date of issue.
Persons registered with a drug dispensary can receive a positive conclusion from a narcologist at least three years after deregistration, provided they get rid of drug or alcohol addiction.
Such certificates can be obtained in advance. For example, go to a narcologist and a psychiatrist in early March, and start seeing other specialists in early December. Some of the work will already be done. A driver's medical examination certificate issued in December will be valid until the corresponding date in December of the following year.
A certificate from a narcologist is valid for one year
Russian legislation states that future or established drivers can see a psychiatrist and narcologist only at the place of their permanent or temporary registration. And situations in which a person does not have registration at his actual place of residence occur quite often. Therefore, in such cases, it is worth thinking about passing a medical commission in advance.
How long does it take to issue a medical certificate?
Certificate form 003-В/у, which is required by traffic police officers to obtain a driving license, will be valid for 2 years in 2021. This is stated in Order of the Ministry of Social Development of the Russian Federation No. 302-N, namely in Appendix No. 2 of this document. A document with a validity period of 2 years is issued for all categories of driver’s licenses - A, B, C, D and E. But there are also certain categories of drivers for whom the document is issued not for 2 years, but for only 1 year:
- young drivers who have not yet turned 21;
- drivers professionally engaged in transporting passengers;
- drivers who have special diseases that in any way limit their right to drive vehicles.
Confusion with the validity periods of certificates for submission to the traffic police is due to the fact that until recently, or more precisely until 2015, all medical certificates were valid for much longer. For most car owners, the validity period of certificates was previously 3 years (now - 2 years), and for special categories of car owners - 2 years (now - 1 year).

In what cases is a certificate required?
In 2021, the list of cases in which drivers had to go to clinics to obtain medical reports was significantly reduced. Thus, from February 16, 2021, it is not necessary to issue a medical certificate if it is necessary to change a driver’s license due to its loss, theft or wear, when changing a surname, as well as when obtaining an international driving license in addition to national ones.

- you need to get a driver's license for the first time, immediately after passing the exam at a driving school;
- the driver's license has expired (10 years);
- the license must be returned after it has been withdrawn by a court decision (for example, due to driving while intoxicated);
- a professional driver has noticed noticeable changes in his health, which must be reflected in a new certificate (if he previously had diseases that impose certain restrictions on driving);
- you need to open a new driver category.

How to get a new medical certificate?
Certificate form 003-В/у is issued in specialized institutions (polyclinics and medical examination clinics), and you will have to pay a certain amount for this. The exception is when the employer pays for a medical examination when hiring a person to work as a driver. Exactly how much you will have to pay to obtain a certificate depends on the specific medical institution and region. On average, this procedure now costs 1000-2000 rubles and more.
It is best to choose private medical clinics rather than government institutions to obtain a medical certificate, because this will save a decent amount of time. If everything goes well, you can see all the doctors even in one day. You will definitely need to see a general practitioner, an ophthalmologist, as well as a psychiatrist and narcologist (the last two doctors are at your place of registration). To obtain some driving categories, other specialists may be required - an otolaryngologist, a surgeon.
To obtain a medical certificate, you will need to present a passport, a previous driver's license (when replacing a license), and a military ID (for men). Previously, it was required to provide 2 photos, but now this is no longer necessary. There are no photographs on modern medical certificates.
How and where to get a medical certificate, cost, list of doctors
You can undergo a medical examination at a state, municipal or private clinic, if it has a license to issue such documents for drivers. Russian car enthusiasts have the right to make their own choice. As a rule, several days a week are allocated for the driver's commission, when all the required specialists are on site. You will need to go through:
- therapist (replaced by a general practitioner or family doctor);
- ophthalmologist;
- neurologist (required for categories C, D, CE, DE, Tm, Tb and subcategories C1, D1, C1E, D1E, for others - upon referral from a therapist);
- otorhinolaryngologist (for categories C, D, CE, DE, Tm, Tb and subcategories C1, D1, C1E, D1E).
Separately, you will need to visit a narcologist and a psychiatrist. These specialists have the right to issue certificates only in state or municipal medical institutions at the place of permanent or temporary registration of the motorist. This could be a dispensary or a specialist in a clinic.
For categories C, D, CE, DE, Tm, Tb and subcategories C1, D1, C1E, D1E, it is necessary to do an electroencephalography; the rest will need it if the doctor writes a referral. A narcologist may refer you for blood and urine tests.
You only need to have your passport with you, and you can get a medical card and a certificate form at the reception. The driving test is usually completed in a day or at most two. Much depends on the queues and schedules of specialists. In private clinics this may take several hours. If there is a need for additional examinations, the time required to complete the commission will increase.
Drivers and candidates themselves choose the location of the medical examination
On the territory of the Russian Federation, the cost of a medical examination for drivers is not established at the legislative level. It will be different in each Russian region. It is most expensive to undergo this procedure in Moscow and the Moscow region, in St. Petersburg and the Leningrad region. In Moscow, for example, the maximum cost will be about 4 thousand rubles. But in Rostov you can pass the commission for 1,700 rubles. Private clinics set their own prices; we can talk about 6–7 thousand rubles.
You can take a driver's test for free only in one case - if the employing organization pays for it. There are no payment benefits provided at the legislative level.
I have passed the driver's test several times. And each time there was no specific cost and time required for this procedure. The commission was held on Tuesdays, Thursdays and Saturdays at the state clinic. The chairman of the commission was always a therapist, as provided for by the Order of the Ministry of Health. A visit to the therapist completed the commission. It was this specialist who made a verdict based on the conclusions of his colleagues and drew up the medical certificate itself. One day, my license was about to expire, and I was completely confident that I had a valid medical certificate, issued only six months ago. But it was not possible to find her. I went to the clinic where I underwent a medical examination and asked to be given a duplicate document. This did not pose any difficulty, although I had to pay almost 400 rubles. I received a duplicate of the certificate, which expired in six months. But I didn’t have to go through the medical examination again. Receiving a duplicate took no more than an hour and a half, including waiting in line.

In what cases will it not be possible to obtain a certificate?
There are a number of diseases that prevent a person from obtaining a driver's license. For example, epilepsy, loss of vision (total or partial blindness), as well as the inability to distinguish colors. They will not be allowed to drive vehicles if a person is forced to constantly take medications that affect his behavior and reactions.
However, for many ailments, a person will still be given a certificate, but with some restrictions. This applies to the following health problems:
- minor vision problems in which a person must constantly wear glasses or contacts;
- absence of limbs or their pathology;
- hearing impairment;
- minor mental disorders;
- poor functioning of the vestibular apparatus.
If you have certain health problems, a person may not be given permission to drive certain categories of vehicles. Whereas he will have no restrictions on driving, for example, ordinary cars. More stringent health requirements are usually imposed on people who want to obtain a category that gives them the right to drive trucks and buses, that is, professional drivers.
